Our Care Home Liaison Nurse works closely with care homes to improve residents’ quality of life with specialist palliative support that facilitates timely symptom management and promotes dignity and autonomy at end of life.
This service is for residents in care homes in West Norfolk.

A Holistic ‘How Can We Help’ Approach
Together with care homes, we provide holistic care that addresses physical, emotional, psychological and spiritual needs in a coordinated manner that involves care home staff, the resident and their family.
Family/carer involvement is prioritised – our support equips them with knowledge and skills as well as emotional and practical advice.
Timely Intervention and Patient Autonomy
Our Care Home Liaison Nurse offers timely support from the start of your referral. Patients are usually seen for assessment within five working days of referring.
With holistic reviews and prompt symptom management, we work with care home staff to prevent crises and avoid unwanted hospital admissions.
Resident autonomy is at the forefront of our approach. Our Care Home Liaison Nurse respects the choices of residents and works with them and the care home team to plan care that meets their goals and preferred place of death, including advance care planning and ReSPECT forms.
Offering Integrated Care
We work closely with GPs, community health teams and hospitals so care feels continuous, connected and seamless, relieving pressure on loved ones.
How to Refer
We operate a no barriers referral system.
Anyone can refer to the service patients likely to be in their last year of life in West Norfolk Care Homes.
We accept referrals for patients demonstrating a progressive decline in their condition, identified as being in their last year of life and who are on the Gold Standards Framework.
