Our Advance Care Planning workshops provides insight into how you can provide person-centred care to individuals with a terminal diagnosis. We guide you through the key aspects of Advance Care Planning, including the main tools and processes used to outline an individual’s wishes for their future care.
Why This Workshop?
Our Advance Care Planning workshop offers insight into:
- Advance care planning, including why it’s important and when to start it
- Understanding the terminology used in palliative care and end of life
- How we can understand typical disease trajectories to inform the conversations around deterioration
We’ll provide advice and information on opportunities to start conversations about Advance Care Planning, as well as how you can prepare for and tackle some of the barriers to talking about end of life.
Who Should Attend?
This workshop is ideal for any professional who wishes to develop their knowledge and skills around quality palliative and end of life care.
This includes professionals working in settings where they will interact with people who are living with long term conditions or are in the last phase of life. We can accept a maximum of 16 participants on this course.

- Why Advance Care Planning matters in your job role
- The main principles of Advance Care Planning
- The importance of terminology and everyone’s understanding of it
- How to use typical disease trajectories to explore with patients where they are in their disease journey
- How to recognise opportunities in introducing Advance Care Planning conversations
- Key communication skills for these discussions
- What resources and support is available to support Advance Care Planning
What Is Palliative Care?
“Palliative care is the active holistic care of patients with advanced, progressive symptoms. The goal of palliative care is achievement of the best quality of life for patients and their families. Many aspects of palliative care are also applicable earlier in the course of the illness in conjunction with other treatments.” NICE, 2004

This workshop is free to health and social care professionals within West Norfolk. This is being funded through the ICB for 2026.
The ICB recognise that financial constraints are inhibiting both the delivery and access to high quality education and have therefore agreed to fund this training.
Places are limited, so we would recommend you book early to avoid disappointment.
